The PHC Airport location at 6221 NW 36th Street, Virginia Gardens, Florida 33166 will become Concentra Miami Airport.
The PHC Hialeah location at 6990 NW 37th Avenue, Miami, Florida 33147 will become Concentra Hialeah East.
The PHC South Dade location at 1448 North Krome Avenue, Suite 101, Florida City, Florida 33034 will become Concentra Florida City.
The PHC Kendall location at 7887 North Kendall Drive, Suite 102, Miami, Florida 33156 will become Concentra South Miami.
The PHC North Dade location at 20535 NW 2nd Avenue
