Item 1.04 Mine Safety – Reporting of Shutdowns and Patterns of Violations.
On February 11, 2026, the Company received a section 107(a) order issued by the Mine Safety and Health Administration (“MSHA”) under the Federal Mine Safety and Health Act of 1977 at its Kokomo Quarry in Kokomo, Indiana. The order stated that a miner was observed standing on an elevated crusher platform being constructed approximately 10 feet off the ground. Although the miner was wearing fall protection, the miner was not anchored to an effective position to prevent a danger of falling, exposing the miner to risk of injuries. Corrective action was taken, and no one was injured in the cited incident. The order has been terminated.
